The Role Purpose

As an
Accredited Clinical Coder here at the Trust, you will be required to analyse and abstract complex clinical information from patient records across all specialties using a wide range of source documentation and translate clinical terminologyinto a coded format in accordance with National Clinical Coding Standards and Trust guidelines.

Key Responsibilities

To analyse and abstract complex diagnostic and procedural information from patient records across all clinical specialties.
To translate diagnostic and procedural information using ICD10 and OPCS classifications and enter coding onto the Trust's PAS via encoding software.
To routinely access all electronic clinical systems available to ensure all relevant details are recorded for each patient spell.
To maintain high levels of concentration for long periods to ensure all discharged patient spells are coded by SUS submission deadlines.
To assist in resolving coding queries as raised by members of the Clinical Coding team, sharing expertise and assisting in the interpretation of complex information ensuring national standards are met.
To regularly liaise with clinical staff from all specialties and discuss and understand complex diagnoses and procedures where it is necessary in order to assign accurate coding.
To regularly communicate data quality issues with clinical systems teams to instigate correction of errors and highlight to management teams in order to improve data quality.
To regularly communicate issues with uncoded discharges via internal reporting processes in the agreed format.
To maintain the levels of coding accuracy as defined by Data Security Toolkit Standard 1 Data Quality and achieve the performance metrics as defined internally by the Clinical Coding department.
To ensure all objectives set at annual appraisals are achieved consistently.
To be aware of, and adhere to, all current National Clinical Coding Standards. It is the personal responsibility of all Accredited Clinical Coders that all changes to coding standards are implemented correctly as per implementation dates, and that all personalresources are updated appropriately.

To implement policies for own work area and propose policy or service changes which impact on the Trust.
Ensure all queries raised by other Trust staff are resolved promptly and effectively, or where appropriate are referred to the Clinical Coding management team.
Attend alone or accompany where necessary the Clinical Coding management team to discuss coding issues with clinical staff maintaining good links to improve and ensure good quality coded data.
To write and send complex coding queries to external classifications services where necessary in order to resolve problems arising from coding complex clinical scenarios.
To actively participate and ensure availability when scheduled as part of the internal Clinical Coding Audit Programme.
To provide mentoring or training to less experienced coders as and when required, including training/guidance on interpretation of coding standards and correct/efficient use of various clinical systems.
To ensure personal timesheets are accurate and regularly updated via Employee Online/eRostering.
Work with those you support to develop a collaborative working partnership which positively contributes to their overall efficiency and role performance.

Education and Qualifications

National Clinical Coding Qualification (NCCQ) (ACC degree level)
Has passed a Clinical Coding Refresher Course within the last 3 years
Clinical Coding Standards Course
Clinical Coding Workshops including:
Neoplasms
Vascular
Obstetrics
T&O
Evidence of ongoing professional development.

Knowledge

Thorough understanding of the latest National Clinical Coding Standards
Detailed understanding of the Payment by Results (PbR)/National Tariff Payment System (NTPS) and Healthcare Resource Groups (HRGs)
Detailed understanding of statistical uses of coded data including Hospital Standardised Mortality Ratio (HSMR)
Indepth understanding of human anatomy
Extensive knowledge of medical terminology

Experience

Practical experience of working in a clinical coding setting using ICD10 & OPCS in a large organisation.
Coding experience in all clinical specialties
Experience using encoder software and PAS systems
Experience using various clinical systems including EPR
Experience working in high concentration/pressure situations maintaining high standards for long periods
