Social Worker Transition Work

West Locality Teams

(Based in Penwith Locality Team)

St John's Hall Council Office Penzance, Cornwall

  • Experienced registered social worker with extensive experience of working with young adults who are transitioning from Children's to Adult Services
  • Ability to take a personcentred, strength based approach
  • Skilled in undertaking assessments, reviews and support planning.
  • Skilled in working with complex situations and ability to have difficult conversations
  • Skilled in partnership working with Children's workers, education and health colleagues and other partner agencies
  • Skilled in finding alternative solutions to meeting young people's needs and enabling them to meet their goals and achieve the best outcomes in their local communities
  • Knowledge and experience of working within a legislative framework for children as well as adults
  • Experience of contributing to EHCPS
  • Knowledge of Mental Health legislation
  • Experience of working within with in Children and Families Services and Adult Social Care
  • Ability to hit the ground running
  • Resilient and ability to adapt in the face of challenging circumstances.
  • Ability to use Microsoft Teams and Mosaic.
  • Ability to work remotely from home as per Government guidelines
  • Need to be able to comply with Government guidelines from 11th November by having a Covid 19 vaccination to enter a CQC registered care home (Vaccination of people working or deployed in care homes: operational guidanceGov.

Within 2-3 weeks 3 months from start date

Full time 37 hours per week


A standard of personal appearance which is appropriate to the nature of the work undertaken, follows operational requirements and which portrays a professional approach.
