Role Overview:

As a Care Coordinator, you will play a pivotal role in our multi-disciplinary team, working closely with patients, their families, and carers to develop personalised care plans.

You will be the point of contact for patients, providing support, guidance, and coordination across various healthcare services. Your efforts will contribute to improving health outcomes, enhancing patient experiences, and addressing health disparities within our community.


Key Responsibilities:


  • Identify and engage with patients in need of personalised care plans.
  • Collaborate with patients and their support networks to develop holistic care plans.
  • Assist patients in managing their health needs and accessing relevant support services.
  • Coordinate care across healthcare sectors and advocate for patients' needs.
  • Monitor and evaluate the effectiveness of care plans, making necessary adjustments as needed.

Requirements:


  • Good general education with a health and social care qualification preferred.
  • Experience in empowering individuals and working with complex health needs.
  • Strong communication and interpersonal skills.
  • Ability to build trust and effective working relationships.
  • Excellent organisational skills and the ability to prioritise workload.
  • Knowledge of personcentred approaches and healthcare systems.
