We are seeking Community Nurses to join our Workforce Solutions team for 'bank' shifts at Provide. A requirement for this role is to have a driver's license and access to a car.
Main Duties of the Job
The Integrated Care teams collaborate with key stakeholders across Mid Essex to identify adult patients with long-term or short-term health care needs in the community. The team develops and implements individual care pathways that cater to acute and rehabilitation needs, ensuring comprehensive nursing care. Collaboration with GPs, social services, and other service teams is crucial for promoting independent living.
We operate on a three-shift pattern from 8:00 am to 11:00 pm, seven days a week, including bank holidays. Geographical locations include:
