About the role


Salary:
circa £25,000 FTE per annum, dependent on experience


Hours: 30 hours per week (work pattern open to negotiation, may consider a split role)

Job Summary:


The post holder will work as part of the multi-professional family support team and will hold a caseload of bereaved individuals.


Key Working Relationships:

Family Support Team
Clinical Nurse Specialist Team
Bereavement Support Volunteers


Duties will include:

Delivery of high quality bereavement care for bereaved adults, as part of the multi professional team.
Conduct initial assessments to identify the needs of the clients.
Select appropriate intervention methods and refer on for specialist support as appropriate.
Deal promptly and sensitively with all enquiries and clients.

Contribute to and support the multi-disciplinary team on issues related to bereavement care, through working closely with other members of St.

David's Hospice Care to ensure consistency of bereavement care and intervention.
Respect confidentiality.
Participate in regular supervision.
Working in bereavement groups.
Participating in hospice education sessions.


Key Areas of Responsibility:

Bereavement Work

To hold a caseload, including some with more complex bereavement needs.
To decide on appropriate responses to meet the bereavement needs of adults accessing the service.
To work closely with colleagues in the Family Support Team to provide bereavement support for those individuals and families identified as being at higher risk
To liaise with the multi-professional team on matters related to bereavement care.
To develop bereavement support groups in line with service demand.
To take part in the ongoing training and support of volunteers.
To participate in in-house staff support meetings and systems.
To participate in Light Up A Life, Twilight Walk and memorial events.
To accept and participate in regular clinical supervision sessions and training to facilitate ongoing professional development.
To contribute to the further development of the Family Support Team and the organisation
To maintain records, in accordance with SDHC policies and procedures.
To provide monthly statistics and be able to support audit, evaluation and research related to the bereavement service.
To promote the work of the service and the Organisation at all times.

Education

Identify and encourage research-based practice in relation to bereavement care.
Prepare and provide learning materials in conjunction with the Family Support Team and Education Department.

Professional Development

To be committed to enhancing own professional development.
To keep up to date with current good practices in bereavement care.
To undertake training and development relevant to the post.
Person Specification

Essential

To be a qualified counsellor, minimum level 4 diploma in counselling + BACP registered
Experience of bereavement care and/or working with individuals experiencing loss
Working knowledge of counselling skills, gained through caseload knowledge
Excellent interpersonal and communication skills.
Excellent written and presentation skills.
Self-confidence, and an ability to work on own initiative.
Ability to consolidate current knowledge with the potential for further development.
High level of motivation and flexibility.
Working as a member of a multi-disciplinary team.
Reflective practitioner.
Self-motivated, flexible and enthusiastic.
Committed to anti-discriminatory practice.
Ability to hold professional boundaries.
