Description
Airedale Cardiac Rehabilitation Service aims to support people and their families following recent heart attacks or expected cardiac interventions such as stents or internal defibrillators.
We also provide support for people prior to and following heart surgery.
Our team comprises of Cardiac Rehab Specialist Nurses, a Healthcare Support Worker and administration support, we work alongside the wider community nurse specialist team.
We are also supported by the therapy services to run our cardiac rehab exercise classes and have access to several of the multi-disciplinary teams.
A cardiac rehab specialist nurse has an amazing opportunity to make a big difference in a patient's life in such a positive way that it is a true privilege.
An essential but very rewarding aspect of the role is to quickly gain rapport with a patient and their families so that we can promote self-care and offer individualised care.
The role is varied and interesting, we offer flexibility to our patients and our employees as much as possible with patient care being offered in their own homes, clinics, community venues, video calls and the hospital.
We are a small friendly focused team who are passionate about promoting self-care to empower our patients to keep their hearts healthy.

We are a national centre for telemedicine and introduced telehealth to the UK offender healthcare sector and also provide the service to patients in their care homes.

Ensure patients receive a high standard of clinical care and that you meet the standards expected from own professional body.
Plan and implement cardiac rehab interventions in partnership with the service user, with the carer as necessary and in consultation with colleagues.
Monitor and evaluate the outcome of cardiac rehab interventions in partnership with the service user if possible, with the carer if necessary and in consultation with colleagues.
Provide advice, teaching and instruction for relatives, carers, and other health professionals.Refer patients to other members of the MDT where appropriate such as smoking cessation, dieticians, and psychology.
Supervise health care students.
Use highly developed communication skills to establish good working relationships with colleagues, service users and carers including the use of agile technologies and SystmOne.
Use verbal and non-verbal communication skills and sensitivity with patients, relatives, and carers to overcome barriers to communicate, e.g.
loss of hearing, language barriers.Follow supervision guidelines and consult with Clinical Lead to ensure optimum outcome for complex patients with clear presentation of complex facts and analysis to enable effective decision making.
Contribute to the planning and development of the Cardiac Rehab Service in partnership with team members, team lead and operational managers.
Communicate effectively within the immediate team, MDT and with other internal and external organisations, e.g. Primary Care, LGI and Voluntary Sectors.Ensure equipment is stored in adherence with to relevant policies and that you are up to date with competencies to use equipment within the Cardiac Rehab Service.
Contribute to the development of cardiac rehab protocols, policies and procedures designed to improve patient care and experience.Adhere to Health & Safety Policies and Procedures.
Personal and Professional Development
Adhere to the Health and Care Professions Council (HCPC) Standards of Proficiency and Code of Ethics and Professional Conduct for own profession and maintain current professional registration.
Maintain own continuing professional development (CPD) by keeping abreast of new trends and developments, use of reflective practice and incorporating learning into working practice.
Support team to participate in relevant internal and external research and quality improvement projects.Establish links with other clinicians specialising in Cardiac Rehab through regional and national groups to promote sharing of good practice and evidence-based research.
To attend BACPR/BASES study days or conferences alongside other relevant training and education opportunities and to feedback to team and students.
~ Participate in the development of risk assessments strategies for the Cardiac Rehab Service.
Attend Cardiac Rehab Clinical Governance Meetings and contribute to the operational planning and monitoring of the service.
Assist with the development and implementation of Trust and therapy wide service quality improvement strategies.
Ensure maintenance of accurate, comprehensive, and up-to-date documentation in line with professional, legal, and service requirements.
Follow Trust and Departmental Policies and Guidelines.
~ Be aware of and comply with the relevant codes of conduct and practice set up by the Health and Care Professions Council (HCPC) or Nursing Midwifery Council (NMC) and maintain professional registration. Any breach of these codes may lead to action by the Trust independent of any taken by the HCPC or NMC
Current UK Driving Licence and access to own vehicle for business use
Ability to travel across a large geographical area
Qualified nurse registered with NMC
Two years relevant experience working at Band 5 level
Relevant and recent experience working in cardiology
Experience of multi-disciplinary team working and working collaboratively with other professions and agencies including statutory, private, and voluntary sector services.
Experience of cardiac rehab assessments and treatments
Experience of working within primary care and community settings
Experience of taking part in or leading on projects/service improvements
Experience of guideline development, implementation, and audit
Experience of providing personalised care and promoting self-care
Knowledge of Cardiac Rehabilitation
Evidence of continuous professional development
Excellent planning and organisational skills
Excellent communication and counselling skills including observations, listening and empathy
Knowledge of cardiac rehab patient management in acute or community setting.
Knowledge of NACR and NICE cardiac rehabilitation guidelines
Specialist knowledge and clinical reasoning skills to support expert clinical practice in cardiac rehabilitation.
