About Us:
Care First Services is a leading provider of semi-independent living accommodation for young people transitioning into independent living.

We are committed to empowering our young people to develop the skills and confidence necessary for a successful transition to adulthood.


Hours per week: 37.5


Job Purpose:
To be responsible for the provision of support for young people year-old.


Main Duties & Responsibilities:


  • To assist the young people in their independent living journey.
  • Provide transport/support to young people as directed in line with support plans.
  • Conduct welcome and induction for young people moving into the property.
  • Update of all paperwork, risk assessments, support plans.
  • Provide ongoing support and monitoring of the young person.
  • Assist in the management of all health issues, physical, mental and emotional needs.
  • Preparation of weekly reports to the social workers.
  • Arrange appointments with various partner agencies.
  • Maintaining regular contact with young people
  • Encourage young people to engage with groups in the community.
  • Provide structure for the young person e.g., register with a college or educational settings.
  • To accurately record, create, maintain and update records in respect of young people.
  • To prepare for and attend meetings as required.
  • To effectively use IT packages.
  • To work effectively and cooperatively across multi-disciplinary teams.
  • To observe and maintain strict confidentiality in respect of the young people and Company.
  • To pro-actively assist in the development of policies and procedures of the Company.
  • To report any housing issues to the maintenance team.
  • To undertake On-Call duties as directed.
  • Able to show flexibility to cover at different times and locations if required by the Business.
  • Attend mandatory team meetings, training and supervisions on a regular basis as requested by management.

  • Communicating effectively within the team, with other teams and across the company.
  • Supporting the culture of outreach services cross the area in our homes.

The outreach worker is required to carry out the duties in accordance with the company Health and Safety policies and procedures.

The outreach worker is required to observe and maintain strict confidentiality in respect of al young people and company information.

The post holder may be required to undertake other duties that may be required from time to time within the general scope of the post.

Any such duties should not substantially change the general character of the post. Duties and responsibilities outside the general scope of this post will be mutually agreed with the post holder.

Qualifications/Requirements
QCF Level - Children and young People or equivalent, or willing to work towards.

Enhanced DBS record.

Willingness to undertake further training.

Current full driving license with business Insurance.

Experience
In a relative social care environment.

Experience of working in a supported accommodation.

In dealing with individuals from ethnic minority groups.

In dealing with vulnerable people from a variety of backgrounds.

Frequent use of computerised admin systems in busy working environment.
