Continuing Healthcare (CHC) - Clinical Team Leader

Permanent 37.5 hours per week

Based in Acton House, Worcester or St Owen's Street, Hereford.

As a new member of the Continuing Healthcare Team you will complete a robust induction within the newly structured and dynamic team.

Participation in Clinical Supervision is considered an essential support feature as well as encouragement for continued professional development. Regular clinical team meetings are utilised for peer supervision and support, as well as in-house education sessions.

The post holder will ensure that they and their team adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols.

You will work towards quality standards and performance as directed by the Clinical Lead and will support the Clinical Lead to ensure that their work stream is planned and managed effectively, assisting in their successful delivery.


The post holder will ensure compliance with all responsibilities associated with processes and protocols, locally and nationally, with regards to the CHC National Framework and any related legislation, including Personal Health Budgets and will support the ICB's safeguarding leads.


You should be confident to provide professional advice and support clinical staff: ensuring that all assessments undertaken and recommendations made for NHS Continuing Healthcare are accurate, needs led, ensuring that all relevant documentation is completed.

There is an expectation to rotate into the role as duty manager as required and act as an interface between administration and clinical teams.
