Description

As a well-established service, The Single Point of Access Team, now have opportunities for experienced staff to join the Team as Senior Hub Coordinators.

We require enthusiastic and motivated individuals to work as part of the Single Transfer of Care Team.

The role of a Senior Hub Coordinator requires individuals who are used to working in a fast-paced, busy operational setting, with changing priorities and challenges.


Supporting effective patient flow across all Hospital Discharges Pathways for admission and discharge, your contribution to the work can be both rewarding and influential as the service continues to develop and evolve.


For all posts there will be a requirement to actively support high quality administrative processes for referral management, coordination and support.


If you have a passion for supporting patients, enjoy the challenge of problem-solving and using your initiative on a daily basis, whilst liaising with many different levels of staff; and patients'/carers, then we would like to hear from you.
