    Job summary

    The Post Holder will be expected to lead, manage and co-ordinate a designated Integrated Neighbourhood Team and will be professionally accountable for the delivery of patient care. He/She will act as a clinical lead for patients with complex health needs supporting the delivery of expert clinical care and providing clinical leadership to nursing team members and facilitate professional specific supervision for other team members.

    Will have continuous responsibility for the management of risk, demonstrating flexibility and adaptability to work safely in different environments. The post holder will be required to work autonomously and manage day-today activities of the Integrated Neighbourhood Team.

    The post holder will be responsible for implementing agreed nursing policies and clinical standards, monitoring the compliance of staff with the above in accordance with agreed criteria.

    Main duties of the job

    Lead other from a base of clinical credibility in order to model and facilitate improved practice and partnerships and demonstrate positive leadership skills identifying and prioritising the workload to ensure a safe operational system, referring to the relevant Cluster Lead as necessary.

    Make a proactive contribution to the future development of the community nursing service and development of integrated neighbourhood teams. Support the Cluster Lead in effective management of financial budgets.

    Respond promptly to any incident or risk relating to patients, self or staff within the team which occur whilst on duty and ensure the timely and accurate reporting of the incident/risk in line with policy.

    Monitor and manage team members sickness in line with relevant absence management policy and record on the recognised Trust system. Liaise with Integrated Neighbourhood Team Leader's, to co-ordinate cover for colleague's caseloads in their absence.

    To manage all annual leave requirements within the team, ensuring service delivery is maintained throughout the year. To take responsibility for own personal development.

    You will take a lead role in focusing team resources to meet the identified KPI indicators as outlined in the Service Specification and identified health needs within the community.

    This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

    The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receivea full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.     Qualifications

    Essential

  • Registered General Nurse with District Nursing qualification or equivalent diploma/degree/master)
  • Management experience
  • Palliative Care course/experience

  • Extensive knowledge in managing End of Life patients and co-ordinating care.
