Hours: 22 hours Wednesday - Friday

Salary Range:
£24,341to £26,216 pro rata per annum

Solent Mind is a partner in the new Primary Level Mental Health Service covering Totton Primary Care Network comprising New Horizons (formerly Forest gate and Totton Heath Centre) and Test vale GP surgeries.

This service will enhance the support which GP practices are able to offer their patients with a range of mental health issues through a collaborative and integrative approach.


This service has been developed to enhance the mental health care which the GP's are able to offer their patients through a collaborative and integrative approach across the three practices and associated services.

We are now recruiting to the following post, which will have a key role within the new service:


In this role you will undertake assessments of patients in order to understand their individual needs and identify the most appropriate support for them, working collaboratively with other partners in the primary level mental health service and associated organisations.
