    Description
    An Excellent opportunity has arisen within Middlesbrough, Redcar & Cleveland Community Services.

    We are looking for skilled and outstanding senior clinicians to join our Urgent Community Response (UCR) service within the Growing the Friarage and Community Collaborative.

    Non-Medical Prescribing is essential.


    You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs.

    You will utilise your advanced clinical skills, non-medical prescribing and case management approach which will have a direct and positive impact on the person's ability to cope with an acute presentation with the aim to reduce unnecessary hospital admissions.


    Experience of working in a community setting, working within an emergency assessment unit or rehabilitation would be an advantage and we welcome informal visits to the areas.

    In the role of the Community Clinical Nurse Practitioner you will be a key member of the unplanned Urgent Community Response Service providing an in depth assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.


    Candidates should hold a full, valid UK driving licence and have access to a car to use for business purposes.

    South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust this training programme has been designed to support our leaders in developing their understanding of leadership and management skills.

    You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role

    After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.

    Qualifications & Training
    To be an independent non-medical prescriber with demonstrable experience
    Registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse.
