    Job summary

    You will work as part of the locality's management team, providing leadership to the wider, locality-based Health & Wellbeing Team and share provision of leadership to the Intermediate Care team within Moor2Sea

    Contributing to the strategic development of Torbay Locality, in partnership with the Deputy Community Service Manager, Community Service Manager and other Professional Leads andto the strategic development of Intermediate Care in partnership with wider Community Service Managers/other Locality Leads

    Working as a supportive and inspirational clinical leader within the integrated Locality team, providing a high standard of service-specific expertise. You will work closely with the primary care networks to ensure that Intermediate Care services are delivered in a safe and effective way; with the outcome of providing an alternative to hospital admission, enabling earlier hospital discharge and preventing early admission to long term care.

    You will also develop and maintain professional working relationships with the Care Homes commissioned to provide Intermediate Care bed-based care.

    Intermediate Care supports the acutely unwell people of Moor2Sea, to manage as safely and as independently as possible in their own homes. Intermediate Care has three main goals:

  • Preventing unnecessary hospital admissions
  • Facilitating timely hospital discharges
  • Preventing the need for long term care packages and care home placements, where possible
  • We pride ourselves on being person centred and flexible, providing high quality, short term interventions.

    Job responsibilities

  • To undertake all aspects of practice as an autonomous practitioner including professional and legal accountability for own work
  • To liaise with various multi professionals, clients and carers, in situations which can involve highly complex, sensitive and occasionally contentious information using well developed communication skills
  • To be responsible for maintaining own competency to practise through CPD activities, maintaining a portfolio which reflects personal development

  • To be responsible for safe, efficient, and effective delivery of Intermediate Care services, promoting team working in order to develop and consolidate a cohesive and supportive environment
  • To lead in the identification and implementation of specific evidenced-based audit and research projects to further services and multi-professional team clinical practice
  • To be responsible for care governance, investigating and resolving complaints and incidents
  • To participate in the development and implementation of policies appropriate to Intermediate Care within the locality
  • To gather feedback in relation to the Localitys Intermediate Care service and use this to inform service delivery and improvements
  • To be responsible for maintaining up to date relevant risk assessments to reduce/eliminate hazards where practicably possible ( health and safety, lone working)
  • To ensure compliance with organisational and departmental policies and procedures
  • To promote the use of tools to ensure a process of continuous quality improvement ( Datix)
  • To have knowledge and understanding of the Mental Capacity Act and Deprivation of Liberty safeguards and support their application within practice
  • Leadership

  • To provide leadership to the Intermediate Care team in the daily MDT meeting, ensuring that targets relating to length of stay and team input are met as well as quality standards and efficient use of resources
  • To be accountable for providing an environment commensurate with life-long learning, and the development of competency frameworks which support the staff development review processes
  • To be responsible for the identification of the professional training and development needs of staff in line with competency frameworks and HCPC/NMC requirements
  • To providing a wide range of human resource services to the team, to include appraisals, sickness/absence, disciplinary/grievance matters, recruitment and selection decisions
  • To be responsible for ensuring that the framework for professional supervision as expressed in the Trust policy is embedded and supported throughout the team
  • To be responsible for the allocation and delegation of service workload and the skill mix management of the team
  • Operational

  • To be responsible for the operational management of the Intermediate Care service
  • To ensure that the Locality Intermediate Care service is developing in accordance with the Trusts strategic view of Intermediate Care. To attend Trust IC Leads meetings to maintain consistency of practice
  • To prioritise and manage the dynamics of an unpredictable working environment through the control and management of effective and efficient use of resources, within budgetary constraints, whilst maintaining high quality, safe delivery of care
  • To consider the need for the management of Intermediate Care capacity across the localities, at times of high pressure within the service
  • To participate in the implementation of local IT processes to allow for effective data collection, in line with local and national guidelines
  • To participate in the development and implementation of both clinical and managerial policies and protocols for the locality
  • To follow guidance set by the Trust in relation to decision making about Intermediate Care funded care
  • To actively contribute to development of Trust wide clinical policies and protocols

  • Effectively utilise budget for Intermediate Care to provide optimal services within available funding envelope, including that spent on commissioned placements and packages of care under Intermediate Care

  • oDegree or Graduate Diploma as a health or social care professional
  • oHCPC, NMC or SW(England) registration
  • oEvidence of continued professional development
