Job summary
You will be a member of a small & friendly team based on the Hillingdon hospital site.
The job involves providing support to service users and their carers in their homes, residential care homes or nursing homes. This may involve supporting people to attend social or therapeutic activities in the community.
The service also supports the older adult mental health wards to facilitate discharge and where possible to avoid admissions by providing support and treatment at home.
Main duties of the job

About us
An exciting opportunity has arisen for a highly motivated and enthusiastic Band 3 Support Worker to join our Older Adults Community Mental Health Home Treatment Service.
The Home Treatment Service (HTS) works with Older adults covering the Hillingdon borough and is based in Woodland Centre in Hillingdon.
The HTS is a needs-led service and work with service users that are physically frail and have a mental health problem. Caseloads are a mixture of service users with long and enduring mental health issues, acute mental health issues or issues relating to memory loss. It is an exciting time to join the team when services are changing to be more dynamic and responsive to the service users within the Borough.
The HTS is a multidisciplinary team, supported by a good administrative system and offers an excellent work environment. The team has good partnership working with local non-statutory Dementia services. The team works closely with our colleagues at The Woodland Centre at Hillingdon Hospital; Oaktree ward, Memory Services and Older Adult Community Mental Health Team.
