We are excited to be offering a new and unique opportunity for a dynamic, experienced and forward-thinking senior registered nurse to work collaboratively with GP Practices, health and care providers and third sector partners in Newport West NCN.
The Place based model of care (PBC), through the lens of collaborative cluster working, is gaining pace in Newport West and to continue this momentum, we have developed the role of Clinical Nurse Lead which is the first of its kind for ABUHB.
The Clinical Nurse Lead will be enthusiastic about delivering effective, seamless care and support services that are based on the unique health and wellbeing needs of our population and most importantly what matters to them.
An important element of the Clinical Nurse Lead role will be to galvanise and further develop collaborative multidisciplinary team (MDT) working relationships in Primary Care, Newport West.
Furthermore, the Clinical Nurse Lead, will have the exciting opportunity of becoming an advocate for the Professional Collaborative for Nursing which is a key primary component of The Accelerated Cluster Development Programme.
