You'll work primarily with our Falls Occupational Therapist, responding to clinic patients, but will also support the Frailty Specialist Nurses & Occupational Therapist in the management of patients on the Virtual Frailty Ward.


The caseload will involve a combination of phone call consultations and home visits under the guidance of the specialist nurses and therapist, with additional administrative duties to ensure the high quality management of patient using these services.


We'll support you all the way with relevant training, giving you the chance to learn directly from our experts across many specialties which should stand you in good stead wherever your career goes next.


Our fab team, known as LIFT, although small, is strong, really supportive and we manage to retain our sense of humour despite some of the pressures we face together and this is mirrored in the wider teams in which we work.

Although a relatively new service - the Virtual Frailty ward has received extremely positive feedback. Patients describe it as 'a lifeline' where staff make patients safe and supportive. This is where we find job satisfaction, knowing the difference we have made to their experience and lives.

To work within the Integrated Frailty Team based at Royal Preston Hospital as part of an expert clinical team delivering quality care within the specialised area and under mínimal supervision from a qualified therapist.

To work in close liaison with the MDT/ patients/ NOK / carers and community colleagues, to ensure effective management and a seamless transition of care, to reduce attendance and readmission risks.

To help to ensure the health, safety and security of users of the Acute Frailty Team, self and colleagues

To support service development and planning

To support the Clinical Governance and Quality agenda within Core Therapies

To support equality of opportunity and access within the Core Therapies for both staff and patients.

To support financial and other resource management within the Core Therapies

To provide and use information to support admin processes and patient care

To directly support the delivery of therapy to patients
