This is an ideal time to join the Integrated Discharge Service as we move into our new home within the Transfer of Care Hub.


The Integrated Discharge Service is growing and we are looking for the right person to join the 3 existing Operational Leads in managing the increasing and evolving team of case managers, coordinators, and IDS admin.

The team plays a vital part in supporting patients to leave as soon as hospital treatment is not required.

The Operational Lead role is a great opportunity to support the team to develop, introduce and design processes, and be a key voice in the Trust in-relation to discharge.

The post holder will provide Operational and Clinical leadership for the Integrated Discharge Service (IDS) ensuring day to day management of the teams is effectively delivered.


Based at Southmead Hospital, the post holder will work closely with the other operational leads, and support the Head of Integrated Discharge, to develop and sustain an effective and progressive IDS that meets the needs of the Trust, and our patients.

Focus areas will seek to enhance patient experience and ability for those that need it to access to acute services, reduce length of stay and readmissions, work with internal and external partners to develop robust and efficient discharge processes, and contribute to ensuring a sustainable IDS workforce.


The role will include change management, the development of effective operational protocols, procedures, processes and management systems to ensure that the case management, coordinator and admin teams continue to deliver effective services to support and promote patient movement out of NBT.


The post holder will be responsible for overseeing work allocation and rotas, reviewing performance and progress, and liaising with partners to ensure that key work areas are fully covered as required.


The post holder will be jointly responsible for undertaking effective recruitment and retention processes, developing training and development models that ensure staff have the required skills and competences to deliver the required level of service.

  • Responsible for the operational management and leadership of a team of case managers. This will include the responsibility for workload allocation, management of sickness absence, disciplinary and grievance issues.
  • Develop and implement effective Standard Operational Procedures within the case manager team to ensure consistency in approach to key tasks
  • Jointly responsible (with other team leads) for undertaking appraisals and setting objectives for all members of the team
  • Supporting teams through periods of change, displaying good change management skills
  • Working closely with internal and external partners to continually develop the Transfer of Care Hub at NBT
  • Responsible for developing operational protocols and systems to ensure effective service delivery that impacts across the Integrated Discharge Service
  • Responsible for development and maintenance of the Board Round Matrix, liaising with IDS partners and wards to ensure cover is effectively maintained.
  • Leading key meetings on a regular basis, including daily Huddles, No Criteria to reside & escalation meetings, liaising effectively with key leads from partner organisations.
  • Working as a key member of the IDS Team Leads group to develop and implement agreed service objectives
  • As part of the Team Leads Group, to jointly support the development of key IDS processes such as discharge forecasting, stranded patient reviews and outlier progress.
