- Work autonomously and as part of a team under the community nursing team lead.
- Assess, plan, implement, and evaluate care for patients with diverse health needs.
- Deliver health and social care programs, including support for individuals with terminal illnesses and families.
- Provide skilled nursing care, from wound management to palliative care.
- Conduct diagnostic tests and procedures, such as venepuncture and drug administration.
- Maintain high standards of professional practice and accountability.
- State Registration on Part 1 of the 3 part NMC register
- Clinical qualifications at diploma/degree level or equivalent experience
- Completion of Trust preceptorship program within the first year for newly qualified staff
- Mentorship Qualification or relevant equivalent qualification (required within 18 months)
