    Job Summary

    Children in Care Senior Nurse/Strengthening Families Practitioner

    Permanent, 30-hour post available

    The Service brings together the following three services:

    • Health Expertise contribution to the Multi-agency Safeguarding Hub
    • The Partnership Plus element of the 0-19 Healthy Child Programme
    • Children in Care Specialist Health Service

    By combining these services, the Strengthening Families team aims to enhance Service Users' health outcomes by:

    • Assigning a dedicated health worker to each eligible family
    • Offering comprehensive support to Service Users and their families
    • Ensuring seamless care transitions and adapting to changing needs
    • Incorporating health services into the Cumbria Safeguarding Hub
    • Collaborating closely with multi-agency local staff
    • Contributing to the development of Integrated Care Communities (ICCs)

    The Strengthening Families Senior Practitioner will oversee clinical care delivery and services in collaboration with families under the guidance of the Clinical Lead Manager.

    Main Duties of the Job

    Children in Care:

    • Achieve competence levels outlined in healthcare guidelines for Looked After Children
    • Promote professional healthcare practices for Children in Care
    • Engage in health assessments and ensure holistic care for Children in Care
    • Provide specialist advice to various stakeholders to enhance health outcomes
    • Facilitate smooth care transitions for children and young people
    • Act as an advocate and partner in multiagency settings
