Community Assessment Unit.
An opportunity for a Band 5 Community Nurse to join the Community Assessment Unit predominately with Urgent Crisis Response Team hours. The post is a hour substantive post covering 5 days per week ( Monday to Friday) for the areas of Southeast Hampshire, Fareham and Gosport.
This role is critical to preventing hospital admissions as part of the Ageing Well agenda, and monitoring patients in their own home as part of the developing virtual ward. The role will be delivered both in patient's own homes and clinic -based care within a Community Assessment Unit and will be working closely with all members of the multidisciplinary team. We are looking for an enthusiastic, caring & motivated professional with excellent clinical & communication skills, who can work autonomously within a scope of practice as a registered practitioner.
The successful applicant will be proficient, knowledgeable, and competent. Educated to Degree or Diploma level in adult nursing registration, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge, and competencies and to be determined by the needs of patients, the service and team.
This job involves a proportionate amount of time driving therefore you will need to be a car owner and have access to a vehicle for work with the correct level of insurance.
Main duties of the job
Upholding all standards as identified by the NMC including the Code of Professional conduct, Documentation and Medicines Management, the post holder will work as an active member of the Community Assessment Unit at Petersfield Hospital and as part of the Urgent Care Response team to provide a high quality, efficient and effective service to patients.
To assist in the delivery of effective assessments for patients with complex health and social care needs. To work alongside Consultant Geriatricians, GP's with Specialist Interest, Nurses, Physiotherapists and Occupational Therapists to provide a high standard of individualised nursing care in accordance with agreed policies and utilising an evidence-based approach. To be responsible for the assessment of patients' needs and the development, implementation and evaluation of planned treatment.
The post holder will be a Registered Nurse and will continue to develop their role through the acquisition of further skills, knowledge and competencies to be determined by the needs of the patients, the service and the team.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
