    Job summary

    We are seeking an enthusiastic clinician who is passionate about admission avoidance to join our Reablement and Flow Team (RAFT) at Queen Alexandra Hospital supporting same day access to emergency care, supporting delivery of care to patients closer to home and preventing unnecessary hospital admission across South East Hampshire. The front door community clinician is an innovative and dynamic way to work with people to support them to receive high quality and advanced level care to access same day emergency care (SDEC), avoid inpatient admission and access wrap around community support. This role is part of an exciting development to deliver admission avoidance services in conjunction with our local partner organisations.

    The role will work closely with Portsmouth Hospitals (PHU), community health services, primary care, adult social care and South Central Ambulance Service to identify patients who can be turned around at the front door with community support. You will offer expert knowledge of community services and how these can support same day turn around of those presenting in ED and SDEC within PHU

    Full induction to relevant acute and community services will be given along with the opportunity to work alongside our community teams in a supernumery capacity to build relationships with and knowledge of our services.

    Main duties of the job

    Support acute Trust with same day discharge from Urgent and Emergency Care areas to improve quality outcomes for patients with a variety of needs, including complex presentations. You will ensure suitable and prompt follow up in the community and improve links between secondary and primary/community services.

    Be responsible for the proactive identification of patients who can be supported by community and primary care services to avoid admission to acute beds

    To lead partnership working between acute and community services alongside the Reablement and Flow Team Lead

    To work with PHU medical, nursing and therapy teams as the expert community voice to support discharges, through directly accessing community services.

    To signpost and enable patients to access appropriate follow-on services in the community, to minimise readmission and to liaise with primary care and community teams for clinically accurate handover and share opinions on next steps.

    To work with the Re-ablement Flow team on site at QAH, and with the wider Transfer of Care Hub (TOCH) team, including Adult Social Care, to help support more complex discharges

    To lead education and staff understanding of the breadth and specialism of community services across partner organisations and build staff confidence in the capability and capacity of community services and their ability to manage greater risk and clinical acuity

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

    Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
