  • This role is categorised as a _
    _roam__ role. Roaming roles are a mixture of home and hub working with, in some instances, a degree of being out and about delivering services to customers. Your working day will be determined by what you need to do and where you need to go to maximise your productivity. This means that you could start and finish your day in different places depending on what you need to do, so we'll support you working from a variety of locations.

The Role:


Our extra care and sheltered schemes offer housing for people aged 55 plus who need housing related support to live independently in their own home.

This position is a part time opportunity for 18.5 hours per week and you'll be based within our sheltered scheme at Gresley Court in Saltburn but will include covering schemes in Eston on occasion and the wider Tees Valley.


Key to the role is the ability organise and complete periodic support plans, demonstrating a flexible, person-centred approach, identifying risk factors, safeguarding concerns and other interventions that enable customers to live happily and independently within an extra care setting.


Working in partnership with families, care providers, health professionals and other organisations is essential to promote healthy lifestyles amongst older and vulnerable customers; whilst also having the ability to arrange social events, in collaboration with charitable organisations to create a settled, friendly and vibrant community.


You'll be flexible, working as part of a team to achieve the outcomes set out by Local Authority commissioner, whilst also demonstrating a positive Team Thirteen approach to customer service delivery.


The Person:

We're looking for someone who is a skilled influencer and communicator, passionate about providing a high-quality service.

Previous experience of working with or supporting older people and an understanding of support plans, risk assessments, safeguarding policies and procedures is desirable.


You must be able to develop and manage a customer-driven service and ensure continuous improvement with a proven ability to analyse and solve day to day problems.


You'll have a full UK driving licence and/or access to a vehicle to be able to travel across the region as required.
