Job summary:


Mid Notts Place Based Partnership is developing Integrated Neighbourhood Teams (INTs) in the 4 localities of Ashfield Mansfield, Newark and Sherwood.

This innovative and exciting initiative will bring together health and Social Care activities to benefit patient care in specific target groups, in line with the recent fuller report.


This newly created role is likely to evolve, develop and shape future health and social care activities in Mid Nottinghamshire.


  • The role is based at Ashfield Health Village for (Ashfield, Mansfield, Sherwood) and Newark
  • Full induction training provided with additional training as appropriate.
  • Depending upon the locality the Patient groups will range from Children to adults to elderly and those from backgrounds of health inequalities
  • The role will work closely with GP leads, local PCNs, Local District Councils social care and voluntary sector
  • The roles will be part of an integrated team within the locality as well across the whole of Mid Notts

Main duties of the job:


KEY RESPONSIBILITIES

  • The post holder will be a key member of the wider Mid Notts Place Based Partnership (PBP) and will provide nonclinical coordination of the Mid Notts Integrated Neighbourhood Teams (INT) to which they have been assigned.
  • The post holder will work closely with the HIIF Clinical Lead, the Programme Manager, Place Transformation Team, INT Clinical Lead for the INT to which they have been assigned and with the INT Clinical Leads for each of the PBP Themes. The Mid Notts PBP Themes are Best Start Plus, Living Well, Ageing Well and Health Inequalities.
  • The post holder will also work closely with other INT Clinical Leads, INT Coordinators and Place (ICB Locality) Team across Mid Notts to ensure the smooth running of the INTs and delivery of the agreed Outcomes.

KEY REQUIREMENTS

  • Educated to Degree level 2:2 (or equivalent experience), Project Management qualification or relevant experience, strong verbal and numeracy abilities with good written verbal communications.
  • Must have an understanding of the background to and aims of Health Inequalities and appreciate the implications of this on engagement.
  • Aptitude for building and maintaining strong and trusting professional relationshipsDemonstrate a good working knowledge of the Mid Notts Place Based Partnership (Plan and Partners).
  • Previous, demonstrable experience of data collection, interpretation and analysis.

The PBP is developing Integrated Neighbourhood Teams (INTs) as a mechanism for delivery which fully embrace joint working across our provider partners (inclusive of VCS as well as statutory bodies) and our communities and residents.


The capacity created via the Health Inequalities Investment Fund (HIIF) (Clinical and non-clinical leadership of the INTs) will facilitate the development of this more proactive approach to tackling Health Inequalities within each neighbourhood and the interventions will help to achieve better outcomes.
