    Job summary

    Health Care Assistant

    An exciting opportunity is available for an experienced Health Care Assistant to join our multidisciplinary team. Withsubstantial experience of working within a caring role, you will work as part of the hospice team to support patients experiencing life changing illness or end of life care needs with understanding, sensitivity and well developed communication skills Whilst primarily based on the Inpatient unit you may be required to work across clinical departments to meet patient need and support your own development

    What we offer

    Salary: WTE £23,419 - Pro Rata: 30hrs £18, £14,

    Working Hours: Hours are worked on a 7 day rota inclusive of days, nights, weekends and Bank Holidays

    TheSuccessful Candidate

    Youwill have the commitment and ability to deliver high standards of holistic careto our patients and their families under the direction of registered nursingstaff and will ideally have achieved the Care Certificate and be educated toNVQ Level 2/3 or equivalent in a related field.

    Job responsibilities

    KEY PERFORMANCE AREAS

    Under the direction of the clinical team/shiftcoordinator support patients and those that matter to them through theprovision of personal care, clinical interventions and wellbeing and enablingsupport

    Deliver high standards of essential nursing careto patients and their families in keeping with care plans developed byregistered practitioners

    Participate in the observation of patients andreport changes in the patients condition to the registered nurse

    Support patients with therapeutic and diversionalactivity while promoting independence within patient limitations

    Work at all times to reduce risk, maximisequality of life and promote patients dignity and independence

    Provide support to families and those thatmatter to the patient as directed by the clinical team

    Communicate effectively with patients andrelatives; be empathic and courteous, treating people with care andconsideration at all times and relaying relevant information in a timely manner

    Ensure own entries in patient records areaccurate and factual and maintained in line with hospice guidelines andstandards of practice

    Recognise and respond to urgent situations,seeking assistance as necessary and relaying any relevant information on thecondition of patients, their care or treatment in a timely manner

    Positively engage with opportunities for directsupport and supervision, the development and maintenance of skills andknowledge

    Work across clinical areas during the span ofduty where this is necessary to meet patient need or for own development needsincluding for example Supportive Care (Hospice At Home) and Support andWellbeing services

    GENERAL DUTIES AND RESPONSIBILITIES

    Maintainappropriate and respectful professional relationships with colleagues and otherstaff

    Exercisegood personal time management, punctuality, appearance and consistent reliableattendance

    Contributeto clinical reports and data collection as required by the Departmental Lead

    Accuratelytake and communicate messages/information using a range of media includingtelephone

    Act as an ambassador for the East LancashireHospice promoting at all times our vision which is to help everyone who comesinto contact with the hospice to make the most of life. We do this bycommitting to our values : to care, listen, respect and work together tosupport people to live well until they die and support those affected by death.

    Ensurethat personal actions and conduct apply with hospice policies and procedures

    Ensureeffective use of hospice resources

    Be responsible for the correct use of nursingequipment and reporting of concerns / faults that may occur

    Participate in general and clerical duties tomaintain a clean, tidy and effective working environment

    Be aware of developments and quality initiativeswithin East Lancashire Hospice using established communication methods

    Work at all times to safeguard the rights of individualspromoting their wellbeing and protecting those at risk from harm, acting on anyconcerns related to safeguarding children and adults at risk in line withpolicy and local guidance

    PERSONALDEVELOPMENT

    Access mandatory training appropriate to therole

    Actively seek to develop own knowledge, skillsand understanding of palliative care using available resources and developmentopportunities relevant to the role.

    Participate in the hospice appraisal system,performance reviews, support and supervision activities to enhance own practiceand provision of care to patients

    Participate in the education and training ofothers where appropriate

    STATUTORY DUTIES

    Ensure the security of information accessed inthe course of your duties ensuring compliance with the Data Protection Act,Caldicott principles and information governance best practice guidance

    Report any hazards or incidents in accordancewith the Health Safety and Risk Management Policy and procedures

    Comply with the East Lancashire Hospice nosmoking policy in all hospice premises and grounds

    Be responsible for maintaining own vaccinationsincluding Hepatitis B immunity (as applicable)

    Ensure that infection control measures arefollowed in relation to your role

    Work at all times to safeguard the rights ofindividuals promoting their wellbeing and protecting those at risk from harm,acting on any concerns related to safeguarding children and adults at risk inline with policy and local guidance

    Work at all times to respect peoplesindividuality, rights and diversity to ensure equality

    WORKING HOURS

    The post-holder will work theirhours flexibly to meet service need s according to a rota of up to any fivedays of a 7-day working week.

    Essential Criteria

    Essential

  • Practical experience of caring for patients or family with life-limiting illness
  • Experience in a caring role
  • Good standard of English and Math GCSE/equivalent
  • NVQ Level 2 in care/equivalent
  • Care Certificate or willing to undertake
  • Understanding/sensitive to the needs of people needing palliative/end of life care
  • Understanding of confidentiality in the healthcare environment
  • Safeguarding adults/vulnerable people
  • Maintaining patient records
  • An enabling approach and ability to empower and promote independence
  • Basic IT and keyboard skills/confidence
  • Ability to utilise the electronic clinical records system
  • Able to relay messages accurately
  • Ability to problem solve
  • Able to provide and receive constructive feedback as a means of individual and service improvement
  • Basic housekeeping skills
  • Flexible, able to work a range of shift patterns including days, weekends, nights and Bank Holidays
