1. Effectively manage and prioritise workload to ensure that deadlines are met while maintaining high standards of written work and achieving effective outcomes.
2. Conduct Assessments/Reviews in a person-centred and holistic manner, focusing on the individual's strengths and preventative options to maximise their independence.
3. Provide support to carers/families as necessary.
4. Ensure that all Assessments/Reviews comply with the Mental Capacity Act and that workers make appropriate judgments regarding service user engagement and advocacy support. Carry out Mental Capacity Assessments and make Best Interest Decisions when necessary.
5. Use the Assessment/Review process to achieve outcomes for service users and hold providers accountable for meeting these outcomes.
6. Maintaining accurate and comprehensive records.
7. Implement changes to support arrangements and present cases to the panel when additional funding is required. Work with brokerage to make necessary changes and pull back unspent personal budgets to ensure value for money.
8. Adhere to the principles and framework of the Care Act 2014, ensuring that Assessments/Reviews are accessible, person-centered, holistic, and focused on wellbeing and prevention.
