    An opportunity has arisen to work within the Community Services Division as an Advanced Nurse Practitioner (ANP) in the Ruperra Ready to Go (RTG) Unit.

    The Ruperra RTG Unit is a newly established nurse led ward based on D6E at the Royal Gwent Hospital, Newport.

    The Ruperra RTG Unit model delivers a nurse led enablement ethos for patients who have been determined as medically fit and therapeutically optimised but cannot be discharged from hospital to the place that they call home due to delays such as availability of a long-term care package or social care placement.

    The post holder will possess advanced knowledge of community hospital patients thus ensuring their effective management and treatment. They will provide direct patient management in partnership with GPs, RTG Unit staff and MDT and will have sound clinical knowledge and highly developed clinical decision-making skills in community care.

    The ANP will be responsible for the provision of effective, efficient and appropriate high quality nursing care by ensuring that each patients pathway is effective and timely.

    The ANP will support the RTG Unit Ward Manager to promote evidence-based practice and will act as an accountable role model.

    Main duties of the job The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

    The post holder will possess advanced knowledge of community hospital patients thus ensuring their effective management and treatment. They will provide direct patient management in partnership with GPs, RTG Unit staff and MDT and will have sound clinical knowledge and highly developed clinical decision-making skills in community care. The ANP will be pivotal in ensuring that patient health needs are met whilst on the RTG Unit through the autonomous provision of diagnosis and the management of patient conditions through assessment, treatment, and the review planning process.

    The role of an ANP is undoubtedly integral to the success of the RTG Unit and the applicant should possess the ability to prioritise fluctuating workloads, remain focused, and be flexible in an extremely busy environment

    A priority of the ANP will be to work collaboratively across healthcare boundaries to develop the RTG Unit service model to improve practice, implementing and evaluating evidence-based standards, guidelines and policy.

    The ANP will be responsible for the provision of effective, efficient and appropriate high quality nursing care by ensuring that each patients pathway is effective and timely.

    The ANP will support the RTG Unit Ward Manager to promote evidence-based practice and will act as an accountable role model.     • Level 1 Registered Nurse - Adult.
    • Current NMC Registration.
    • Master's Degree in Clinical Practice
    • Independent prescriber.
    • Completed ABUHB ANP competency framework.
    • Advanced Life Support skills.
    • Awareness of the ABUHB Deteriorating Patient Policy.
    Desirable
    • Teaching, Mentorship or Coaching qualification
    • Completion of All Wales portfolio (To be completed within 2 years of being in post)
    Experience Essential
    • Evidence of on-going personal & professional development
    • Significant understanding of the Transforming care and 1,000+ agenda.
    • Significant understanding of Research and Evidence based practice
    • Significant understanding of the Free to lead Free to Care agenda.
    • Safeguarding training to level 2 and 3
    • Significant experience as a registered practitioner.
    • Extensive experience as a Band 6.
    • Significant experience of Clinical Audit
