MAIN PURPOSE OF THE JOB
1.
Early identification and prevention:
Taking a whole family approach, identifying, engaging and supporting children and young people identified as but not limited to vulnerable and 'at risk' of ASB, youth crime, displaying irresponsible adolescent behaviour and other associated risk factors, aged 5 to 19 (and up to 25 with SEND) and assess their level of need in line with the 'Early help continuum of need and thresholds of support' using appropriate tools such as Statutory / Early Help
Assessment / Education and Health Care Plan / intervention plans for those who have needs at level 1 and level 2 of the continuum.

Targeted early help interventions:
To undertake Statutory / Early Help
assessment, creating, monitoring and reviewing early help intervention and support plans for children and young people 'at risk' / vulnerable, taking a whole family approach.
To work closely with children, young people and parents and where appropriate with statutory, community and specialist / other agencies such as but not limited to Early Help, Children and Family Services, Community Safety Partnership and the Youth Justice Service in providing joint interventions to address needs identified, achieving positive outcomes in their personal, social, emotional, health and education development and progression.
3.Partnership working:
Ensure a co-ordinated approach is undertaken to offer diversionary and preventative interventions, ensuring effective integrated working models are developed, maintained and reviewed, maximising engagement, participation and learning opportunities that support children, young people and the family in their journey that safeguards their welfare and well-being, achieving positive outcomes that supports progression routes.
4.Caseload management:
To use youth work principals and case work
methodologies to build resilience, challenging negative attitudes and perceptions, informing decisions and empowering children, young people and their families through a mixture of undertaking statutory / Early Help Assessments, devising support plans, engaging in personal development opportunities and positive activities including directly delivering short-term individual and group interventions/ sessions that lead to positive outcomes in their personal, social, emotional, health and educational development and enhances their ability to develop life skills, learn, work and transition to adulthood.
5.Lead professional:
As required, to take on the Lead Professional role by ensuring that cases are co-ordinated and risks are managed to safeguard the child, young person and their family, and to act as the single point of contact for the range of different professionals involved, ensuring that the step up and step down of cases between professionals and services when required occurs seamlessly.
DUTIES & RESPONSIBILITIES
- To work primarily with children, young people and their families aged 5 to 19
- To manage a caseload of children and young people identified in need of support, being flexible in providing a person-centred approach supporting and involving parents / carers and their families in their child/ren's learning, developing positive and trusting relationships, providing one to one and group support through monitoring and maintaining regular contact, negotiating and advocating for and on behalf of children and young people.
- To manage a mixed caseload, providing wrap-around support through early identification, prevention and intervention reducing risk factors associated with children and young people. To remove barriers to participation & engagement in Education, Employment & Training, integrating whole family, restorative and Youth work Practices as key approaches to engaging in positive learning activities.
- To maintain, develop and sustain regular engagement to demonstrably safeguard
welfare by preventing escalation into statutory services by improving participation, aspirations and outcomes in their personal, social, educational and emotional development.
