- Ensuring medicines reconciliation is completed for patients admitted onto the assessment unit
- Clinically reviewing medication and working with doctors to ensure patients are on the most appropriate medication
- Working as part of the MDT
- Supporting the training and development of pharmacy staff, junior doctors and other allied health professionals
- Providing clinical support to the Same Day Emergency Care (SDEC) unit
- Undertaking audit and project work as part of service review/service development
- Relevant post-registration experience
- Experience working on a busy hospital ward and working as part of an MDT
- Experience of participating in audit activities
- Vocational masters degree in pharmacy
- Registered with General Pharmaceutical Council (GPhC)

Job summary
The Pharmacy department are looking to appoint a Clinical Pharmacist to join our successful Adult Assessment Unit (AAU) team. AAU provides an exciting, varied environment to work in and allows you to utilise your clinical and interpersonal skills.
You will work as part of the multi-disciplinary team (MDT) alongside consultants, junior doctors, nurses and AHPs, optimising medicines and having a positive impact on the patient experience. In addition to providing a comprehensive pharmacy clinical service, there will be opportunities to participate in MDT meetings, service improvement projects and training of pharmacy staff and other AHPs.
Candidates must be well-rounded pharmacists with excellent team-working and communication skills to provide a patient-facing, high-quality 7-day clinical pharmacy service. Working patterns will involve 12 hr shifts (8am to 8pm or 10am-10pm) over a 7-day week, including bank holidays. In general this results in working 3 or 4 shifts per week alongside another AAU pharmacist and medicines management technician.
