This role will be required to work across all 3 hospitals of the Trust, Royal Hampshire County Hospital, Basingstoke and North Hampshire Hospital and Andover War Memorial Hospital.


The post holder will play a key role within the Trust in the development and delivery of high quality clinical services, coordinating effective day to day performance management across an agreed portfolio of services and will lead a portfolio of transformation and improvement programmes.

They will provide leadership and management for the Integrated Discharge Team overseeing complex discharge and in support of admission avoidance and High Impact User plans to reduce LOS.

They will also support the Head of Patient Flow and Virtual Health to deliver further improvement and ensure service monitoring contributes to the wider performance and assurance framework for the Trust.


The post holder will ensure a smooth and effective inpatient discharge process and will also be responsible for the interface of discharge planning and services with the discharge tam and key staff including consultants, heads of nursing, matrons and therapists, community providers and health partners and voluntary organisations.


The role will be responsible for the day to day development and performance management of the discharge team, discharge lounge and interface with Immediate Care Services.

Deliver excellent customer service to all patients/visitors and staff at the Trust


Develop effective ways of working and create strong partnerships and relationships with all stakeholders to support the implementation of the Government's policies on health.

Act as an advocate for the Trust and its contribution to the health service arena through creating effective partnerships and relationships with the internal and external stakeholders

Comply with governance structures in keeping with the principles and standards set out by the Trust


Specifically the post holder will support the Head of Patient Flow for ensuring that effective systems and processes are in place across the Trust to support the appropriate and timely discharge of patients from the acute setting out to the community.
