BBS Recruitment is currently recruiting an experienced Support Worker in a temporary to permanent position for our client in Witham, Essex.
JOB PURPOSE
To provide high quality individualised outcome focused care and/or support to service users.
To enable service users to improve their quality of life, including mental and physical.

ACCOUNTABLITIES & RESPONSIBILITIES
To provide unsupervised person centred care and/or support for service users to the
required standard in areas including but not limited to personal care; maximising physical.

To lead on the delivery support, support plans and risk assessments and to have
responsibility for formally signing off the plans.
To facilitate person-centred planning meetings. To utilise a range of approaches including
collaborative multi agency working.
To work with the people that we support to devise effective positive behavior support plans.
and/or recovery tools which are relevant to the service.
To develop support plans, risk assessments and risk management tools as required and to
support service users to meet their needs, goals and aspirations and support agreed
outcomes within the support plan / recovery star.
To take responsibility for organising and chairing formal support plan and risk management
review meetings and evaluations. This will include contacting family members, professionals.
agencies and other relevant stakeholders.
To have responsibility for signing off the support plan and risk assessment review process
To be responsible for liaising with and updating professional agencies on the progress of the
support planning / recovery star process.
To treat service users with dignity and respect, communicating effectively to meet
their diverse needs.
Requirements:
2 Years experienced

Part-time hours: 35 per week
Salary:
£11.24-£12.00 per hour
