EDP Drug & Alcohol Services have an exciting full-time opportunity to join our community team as a Hospital Liaison Worker.
As a Hospital Liaison Worker, you will work from the RD&E Hospital in Exeter and will be responsible for establishing a high-quality drug and alcohol hospital liaison service for patients, providing appropriate assessments, interventions and co-developing multi-agency discharge plans and pathways to reduce relapse and hospital re-admission.
The Hospital Liaison Worker will operate within a multi-agency environment communicating and working closely with relevant hospital departments (e.g. Mental Health, Hepatology & Gastroenterology, A&E, Maternity) and other community services (e.g. GPs, outreach, housing and community justice services).
You'll provide advice, guidance and training to healthcare professionals in recognition, treatment & management of drug and alcohol related problems, including the delivery of brief interventions.
You will carry a caseload of service users, providing effective key working and undertaking collaborative recovery planning, supporting them to participate in a range of recovery, psychosocial and support activities appropriate to their phase of recovery.

In addition, you will also:_
- Ideally you will have experience working in a similar field or environment
- Experience undertaking assessments and delivering a range of evidence based substance misuse psychosocial interventions.
- Experience working in a hospital setting and knowledge of hospital discharge planning would also be particularly advantageous.
- You'll work well in a multiagency environment, able to communicate effectively to ensure a safe transition to the community.
- You'll be able to demonstrate excellent organisation and time management skills and will be confident managing your own priorities.
