Description
We are excited to offer an opportunity a Enhanced Care Co-ordinator to join the team. Enhanced Care and the Integrated Discharge Team are based at North Tees.

You will work closely with the Patient Flow Team and ward staff as well as local partners including social care and voluntary agencies to support care transitions.


We require a person who has excellent leadership and communication skills who is confident working across professional boundaries and agencies.

You will need to be flexible, adaptable and a good team player with the ability to work as part of a team as well as independently to provide a whole person approach to the care and management for patients within this setting.


This is an exciting opportunity to work with Enhanced Care and Integrated Discharge Team to develop services ensuring excellent service delivery to patients through the system.
