    Job summary

    Tees, Esk and Wear Valleys NHS Foundation Trust are currently looking for an enthusiastic support worker to join our Care Home Liaison Hub Team in Bishop Auckland. The post holder will work with the hub team and alongside the wider Community Teams for older people within County Durham and Darlington, delivering physiotherapy and speech and language therapy (SLT) interventions.

    A key focus of this role will be to support multidisciplinary working with the wider community team to help embed SLT and physiotherapy provision. As a support worker, after a period of training you will be expected to assist, deliver and review interventions to patients, care staff and families.

    The post holder should be motivated and excited by the opportunity to represent the SLT and physiotherapy service, where our role and input are valued as part of the patient's holistic care planning. The post holder will have previous experience of working with older people. You will also have excellent organisational, verbal, literacy, computing and observational skills.

    As a new member of staff, you will have support to develop your role through access to regular appraisal, quality clinical and management supervision, and excellent training & development programmes.

    Main duties of the job

  • Works with registered AHP therapists, assisting in patient assessments, treatment/therapy and monitoring of interventions.
  • Assist the Physiotherapist with hands on patient moving and handling and supporting and reviewing exercise plans formed by physio and patient.
  • Information gathering to help produce communication aids and production of these aids to support the patients communication with care staff, family, professionals etc.
  • Review current interventions and plans in place.
  • Engage in daily team huddles.
  • Access regular clinical and management supervision.
  • Complete annual appraisals and work towards goals and objectives set.

    An exciting opportunity has arisen for a competent, confident, and enthusiastic Band 3 Support Worker to work alongside the physio and SLT AHP's in our team. The Care Home Liaison Hub Team is based within mental health services for older people covering the 116 care home within County Durham and Darlington and is based in Auckland Park Hospital, Bishop Auckland.

    The multi-disciplinary team works with older people with a behaviour that challenges likely relating to a cognitive impairment, and their families to deliver non pharmacological interventions to the patient.

    We are looking for a support worker who works closely with the physio and SLT AHP who can work flexibly to meet service need and has the vision, energy and inspiration to support in the delivery of creative, imaginative solutions to delivering interventions to our older people, carers and families.

  • Works with delegated service users who remain the responsibility of the registered AHP therapist, reporting back in a timely and appropriate manner. Patientsmay demonstrate behaviours that challenge or have difficult family situations of relevance.
  • Delivers therapeutic interventions, designed by registered staff, to groups or individuals, observing and monitoring service user progress and making adjustments to interventions (within established protocols).
  • Encourages patients/carers in a compassionate manner, to actively participate in their own therapy to support them in achieving their identified goals.
  • Provides information and practical advice, within scope of own competency, to patients and carers.
  • Participates in health promotion activities
  • Provides information and updates at clinical meetings on service user progress, under the guidance of a registered AHP therapist.
  • Responsible for recognising the signs of service user harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding of adultswho may be at risk and reporting all such concerns in accordance with Trust Safeguarding procedures.
  • Responsible for maintaining accurate and comprehensive patient records using PARIS/CITO or other electronic systems approved by the Trust and in accordance with the Trust and professional record keeping standard
  • Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
