We have a vacancy for a Band 7 Acute Liaison Practitioner to work within the Surrey Continuing Healthcare Team

As an Acute Liaison Practitioner you will work with Frimley Park Hospital.

The post is suitable for an experienced Band 6 looking to progress their skills or an established Band 7 who wishes to specialise or gain experience in the Continuing Healthcare setting.
