The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.
Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.
The post holder will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the CYP National Framework for Continuing care implemented and complied with, according to the principles and processes.
The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

- Communicating information, risks, issues and dependencies, including briefings and reports
- Maintaining constructive relationships with a broad range of internal and external stakeholders
- Working closely with care providers involved in delivery of the service.
- Providing coordination of, and participating in, meetings and providing information, advice and support as required
- Assisting and supporting individuals and representatives in relation to the CYP Continuing care process
- Maintaining uptodate knowledge of legislation, policies and guidance
- Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service
- Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.
- Contribute to the maintenance and monitoring of service standards around quality.
- Supporting the CYP Continuing Care team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery.
- Keyboard skills
- Access to vehicle for occasional travel to visit individuals, providers and other sites.
- Managing a caseload of patients
- Ensure the principles and duties of safeguarding are holistically and consistently applied.
- Supporting the arrangement of services required to meet the needs of individuals who qualify for CYP Continuing Care and for the healthcare element of a joint care package.
- Fostering transparency and a personcentred approach with children & young people, families and representatives
- Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied.
- Working within a team of CYP Continuing Care Assessors, ensuring a highquality service is delivered, supporting staff to achieve continuous improvement.
- Ensuring Continuing care guidance is followed, and services are delivered to maximise patient safety.
- Actively supporting the CYP Continuing care team in working towards goals and objectives
- Contribute to maintenance and monitoring of service standards around quality.
- Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies.
- Knowledge of Personal Health Budget process
- Mentoring and supervision of students
