    This is an exciting opportunity for a Clinical Lead Occupational Therapist with expert knowledge in the management of patients in primary and community care to work within South Ceredigion as an integral part of the cluster teams supporting primary care provision. Crucially this model of occupational therapy provision can:

    Reduce contact with General Practice by resolving underlying issues that are the root cause of regular contacts with health and social care.

    Maximise/ maintain independence in preferred home environment and proactively resolve health and social issues at an early stage, minimizing crisis situations that result in hospital presentation/admission.

    Release professional capacity by focusing intervention on enabling people to maximize their potential, prevent ill health and dependency.

    Work collaboratively within the developing Locality Teams.

    Main duties of the job A Clinical Lead Occupational Therapist in General Practice will:

    Be identified as a clinical expert and have a high clinical profile ensuring the provision of effective and efficient care.

    Work as a highly skilled senior practitioner with expert knowledge, and work in partnership across Health Board, Local Authorities and with specialist services to deliver timely, seamless, person centred, coordinated care.

    Be responsible for gathering/ inputting activity data accurately and regularly, and collect, analyse, interpret and present data for clinical audit and reporting purposes.

    Act as a practice leader, have the ability to develop relationships with key stakeholders, manage their own workload and work across professional, organisational, agency and system boundaries, to improve services and develop practice.

    Participate in networks, locally, regionally, and nationally.

    Assess and manage risk and proactively challenge others about risk.

    • Therapeutic handling and risk assessments in relation to patient care including severely disabled patients
    • Experience in staff development e.g. supervision
    • Experience of appropriate delegation and supervision
