Description
Are you an experienced social worker, occupational therapist or community mental health nurse who would like the opportunity to be part of a developing and innovative integrated Community Mental Health Service?
Telford and Wrekin CMHS provide assessment and evidence based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.
We also keen to incorporate a trauma informed psychological approach which aims to increase people's independence, working towards an improved quality of life.
We are looking to focus this particular role on working alongside the wider team, as well as our inpatient colleagues, to improve people's experience as they move from being in hospital back to their communities.
Delivering a high quality clinical service to patients and their families.
Delivering assessments and interventions, in accordance with an agreed care plan, to an identified group of people experiencing complex mental health related difficulties.

Registered mental health nurse (NMC registration) or Occupational Therapist (HCPC registration) or Social Worker (Social Work England)
Post-registration experience, evidence of formal courses and training at diploma/degree level.
BSc degree in nursing or equivalent level
Evidence of post qualifying training - ie Non - medical Prescriber or psychological intervention qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT, systemic training
Evidence of extensive post registration experience with working with adults experiencing mental health related difficulties
Experience of working with people who have difficulty in engaging with services and have complex mental health needs
Experience of working in community setting and making decisions autonomously
