Social Prescribing involves helping people who have been referred or "prescribed" by their GP, Adult Social Care or other health professional to social, recreational and non-medical support within the community to help them to improve their health and well-being.
We are recruiting for an additional Link Worker to join our growing team.The role involves receiving referrals, carrying out assessments and then linking the person referred to appropriate community services or activities, following up and offering support as appropriate.
To be successful in this role you will need to have experience of working with people in a support role, experience of advice work would be an advantage.
You will need to have an understanding of the issues that individuals referred may be facing, and be able to work with a wide range of people in an empathetic and non-judgmental way.
You will be organised, able to work independently and manage your time well to ensure you are able to meet challenging workloads.
You will need to be a confident IT user.
The role offers the opportunity for remote working, but you will be required to work from our office in Guildford, as well as in GP surgeries and community settings when necessary.
Closing date is Wednesday 27th March 2024.
Interviews will be held on Thursday 4th April face-to-face at our office in Guildford.
Please note this will be the only interview date offered.
