My role as a Community Psychiatric Nurse (CPN):
I handle a caseload of between 25-30 patients and working within my role to complete the following tasks for patients, managing my caseload, working autonomously and flexibly.
• Assessment and Care Planning:
o Conduct comprehensive mental health assessments of patients in my care and other patients allocated on duty, assessment clinics or ad hoc allocations.
o Develop and implement individualised care plans based on patients' specific needs.
o Regularly review and update care plans to ensure they reflect the current needs of the patients.
• Medication Management:
o Administer and monitor prescribed psychiatric medications, ensuring patients adhere to their medication regimens.
o Educate patients and their families about the purpose, side effects, and potential interactions of psychiatric medications.
• Crisis Intervention:
o Provide immediate support and intervention in mental health crises, de-escalating situations to prevent harm whilst in the community.
o Collaborate with emergency services and other health professionals during crises to support patients and staff alike.
• Therapeutic Support:
o Offer counselling and therapeutic support to individuals and families, including talking therapies and psychoeducation.
o Facilitate group therapy sessions focused on mental health recovery, coping strategies, and social skills development.
• Liaison and Advocacy:
o Act as a liaison between patients, their families, and other healthcare providers, ensuring continuity of care.
o Advocate for patients' rights and needs within the healthcare system and the community.
• Risk Assessment and Management:
o Conduct risk assessments to identify potential harm to the patient or others and implement appropriate risk management strategies.
o Monitor and review risk factors regularly, adjusting care plans as needed.
• Health Promotion and Education:
o Provide education to patients and their families on mental health conditions, treatment options, and self-care strategies.
o Promote mental health awareness within the community to reduce stigma and encourage early intervention.
• Multi-disciplinary Team Collaboration:
o Work closely with psychiatrists, social workers, occupational therapists, and other healthcare professionals to provide holistic care.
o Participate in regular case conferences and team meetings to coordinate care.
• Record Keeping and Documentation:
o Maintain accurate and up-to-date records of patient reviews, assessments, care plans, and progress notes.
o Ensure documentation meets legal and organisational standards.
• Support for Transition and Recovery:
o Assist patients with transitioning from hospital to community care, ensuring they have the support they need.
o Promote recovery-oriented practices, helping patients achieve their goals and improve their quality of life.
• Supervision and Training:
o Provide supervision and support to junior nursing staff or students.
o Participate in ongoing professional development to stay updated on best practices in mental health nursing.
