My name is Jayati Bahuguna and I am a Trust Grade ST1 SHO and have recently relocated from London to join my partner who has joined Specialty Training as ST4 in Pediatrics at NUH. I have been working with the NHS for 1.5 years with experience of working in General medicine, Acute Medicine, Nephrology & Care of Elderly at various Hospitals such as: The Queen Elizabeth Hospital Kings Lynn, Whittington Hospital London, Hillingdon Hospital London, King George Hospital London and Queen Elizabeth Hospital Woolwich London.
Previously, I was in India where I have completed my MBBS and my postgraduate degree MD in Hospital Management and worked as a Senior Resident Administrator at AIIMS, New Delhi the apex medical institution in India, is also the largest Public Sector Hospital. I have over 4 years of experience in clinical processes, standardisation, Accreditation for Quality care, Patient Safety, Infection Control and healthcare regulatory compliances.
Experience
- Locum ST1 Trust Grade SHO London (April 2023 – July 2023) –
I have been working as an ST1 grade SHO in Acute Medicine and General Medicine in various Hospitals in London which include – The Hillingdon Hospital, King George Hospital, Whittington Health NHS Trust and Queen Elizabeth Hospital Woolwich. My responsibilities included admitting, assessing and managing acute medical conditions. Perform thorough clinical examinations, order diagnostic tests, interpret results and liaise with on call Registrar/Consultant to ensure comprehensive patient care. Respond to medical emergencies, perform various procedures such as IV lines, urinary catheters, arterial blood gas sampling and ascitic tap. Assist with advanced procedures like chest tube insertion and lumbar punctures. Maintain clear and accurate patient records, communicate effectively with patients, families and caregivers about diagnoses, treatment options and prognosis. Provide mentorship and guidance to junior members of medical team. Effectively manage high stress situations, staying composed and provide clear directions to healthcare team. Participate in handovers and ensure smooth transitions of care during shift changes.
- ST1 Trust Grade SHO since November 2022 Nephrology, The Queen Elizabeth Hospital King’s Lynn NHS Foundation Trust (01 November 2022 – 03 April 2023):
- My work on Nephrology included seeing patients in Outpatient clinics and Dialysis - which entails history taking, examination, investigation review, medication review, and discussion with Consultant for patient advice and follow-ups. Nephrology Clinics include New patients, follow-ups, low clearance clinics, transplant clinics, diabetic endocrine clinics. I also did Ward rounds, QA meetings with Consultant in Dialysis, In-patient follow-ups along with green card referrals which included accepting patients with AKI/CKD on different wards and advising on the treatment. I also liaised with the Nephrology teams at different hospitals like Cambridge University Hospital for urgent patient referrals. I actively participated in scheduled medical teachings for undergraduate medical students and grand round meetings.
- Clinical Fellow FY2 , Acute Medicine and Nephrology, The Queen Elizabeth Hospital King’s Lynn NHS Foundation Trust (10 May 2022 till 30 October 2022)
- My work in Acute Medicine included working on A&E, Same day emergency care and Acute Medical Unit. I work with patients of all ages with a wide range of clinical problems and interact with numerous colleagues including consultants, emergency department staff, critical care and other medical specialists.
- Having done my ALS I have gained significant confidence in assessing acute presentation of medical illness along with managing and formulating a plan, as well as discussing with my seniors to ensure appropriate management of my patients. I follow standardized patient pathways to maximize safe and effective care, providing leadership skills within an acute medical unit, participating in multi-professional systems to optimize care of patients requiring more intensive levels of care along with the wards.
