    Job summary

    An exciting opportunity has arisen to recruit a Clinical Pharmacist to join our Primary Care Network. We serve a population of 90k patients across 8 practices covering Ardwick & Longsight Network. The role will be based at one practice - Dickenson Road Medical Practice

    The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service.

    Main duties of the job

  • Work as part of a multi-disciplinary team in a primary care setting.
  • Provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices.
  • Provide clinical input on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
  • Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload

    Job responsibilities

    To assist the network prescribers in achievingevidence based, costeffective and high quality prescribing

    Lead on the management of the prescribing budget,prescribing initiatives, QIPP, and prescribing elements of QOF, Manchester Standardsand GM quality and medicines management targets.

    Provide expert pharmaceutical advice to the networkpractice clinicians, nonclinical staff and patients as required, in orderto promote safe and effective use of medicines

    Review, develop, implement and quality assure thesafe and efficient prescribing processes, including Repeat Prescriptionmanagement

    Qualifications

    Essential

  • Qualified Pharmacist
  • Member of General Pharmaceutical Council
  • Clinical Pharmacy Diploma or equivalent experience
  • Evidence of commitment to CPD
  • Desirable

  • Prescriber or willingness to train as a prescriber
  • Experience

    Essential

  • Minimum 2 years relevant post-registration experience, providing clinical pharmacy services in a range of settings hospital, community, primary care
  • Experience of working with other healthcare professionals
  • Experience of clinical audit
  • Desirable

  • Experience of developing guidelines
  • Experience of project management
  • Experience of teaching and training
  • Knowledge
