Job Purpose:

The post holder will be responsible for management and co-ordination of the patient caseload and discharge case managers


This will include:

  • Provide business support to the Integrated Hospital Discharge Team.
  • Manage the coordination of resources to adequately cover all whiteboards this requires knowledge and understanding of caseload pressures according to ward areas
  • Advise and provide support to Discharge Case Managers (DCM's) to ensure they adhere to process and discharge standards according to Care Act and National Discharge Guidance
  • CCoordinate all Complex Discharge referrals linking previous support packages to support MDTs in discharge planning
  • Manage the day to day recording and verification of delayed transfers of care project/service.
  • Manage failed discharges and find appropriate solutions.
  • Manage the day to day recording, verification and reporting of Delayed Transfers of Care
  • Ensuring the delivery of seamless high quality service to patients and clinical teams


Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS at Risk staff throughout Devon who is able to display recent relevant experience as dictated by the Person Specification.

  • Provide complex and sensitive information to discharge case managers, Executives, Nurse Managers, at all levels within UHP and CTC.
  • Support senior management in discharge planning meetings as necessary, in the absence of the Support Manager, and compile and distribute etc.
  • Liaise with community teams in the Health community and Local City Councils.
To manage expectations and when necessary implement the reluctant discharge process.
