The Patient Safety Officer will assist the Head of Patient Safety and Risk and the Associate
Director of Quality - Safety and Risk in the promotion; delivery and monitoring of patient safety and risk activity across University Hospitals Coventry and Warwickshire NHS Trust by:

Providing comprehensive support and facilitation to specialty teams on all aspects of patient safety and risk across University Hospitals Coventry and Warwickshire NHS Trust.

Working alongside healthcare professionals to co-ordinate information and reporting processes within the Incident Management Policy, the Investigations & Root cause analysis (RCA) Procedure and the Risk Management Strategy


The role will work within the Patient Safety Team to ensure incidents are investigated and reported within national and locally set timescales.

It will require you to take a proactive approach to the management of incidents across the Trust, ensuring that learning from incidents and trend analysis is fed back positively to change practice.

The role supports senior members of staff across wards and departments to enable timely and high quality investigations, and provides day to day guidance to staff involved in incident investigation handling within the Trust.

  • In conjunction with the Associate Director of Quality and the Head of Patient Safety and Risk facilitate the process of Patient Safety Incident (PSI) reporting, ensuring that investigations are undertaken and action plans developed, implemented and monitored in line with Trust
  • Instigate any follow up action required to further investigate clinical adverse events, carrying out root cause analysis in conjunction with relevant clinicians and managers where Facilitate the release of information to Patients or relatives in accordance with the Duty or Candour or being open policies as appropriat
  • Maintain and interrogate the Trust-wide risk management database, to monitor the progress of PSI, and undertake production of monthly and ad hoc reports for specialty teams on the number, nature and outcome of PSI reported, highlighting trends and action
  • Support the Safety & Risk Team in facilitation of root cause analysis for serious incident investigations by facilitating meetings, ensuring that the relevant documentation is available for the meeting chair person, advising the chair on the RCA process, taking notes during the meetings, assisting the chair with the content of the investigation report and following up any resulting actio
  • In conjunction with the Head of Patient Safety and Risk, the Complaints department and the Legal department monitor the progress of actions arising from adverse events, complaints, litigation and health and safety incidents to ensure changes in practice take place where necessary.
