- Deliver high-quality, evidence-based care in Stroke Rehabilitation.
- Lead the development of clinical practices within the Integrated Community System (ICS).
- Act as a key liaison among the Older People's and Community Directorate (OPAC), CPFT, and external partners to innovate and improve stroke rehabilitation pathways and services.
- Support service delivery with flexibility and adaptability.
- Must be a qualified Advanced Clinical Practitioner with relevant experience in stroke rehabilitation.
- Ability to travel independently across work localities a valid driving license and access to a car for work purposes are essential.
- Demonstrated ability to work effectively in a multidisciplinary team.
- Excellent communication and leadership skills.
- Current HCPC/NMC registration
