Sanctuary Personnel is currently looking for an experienced Band 6Senior Community Mental Health Practitioner (CPN) to take on a new ongoing contract role.
58per hour (assignment rate)Contract Type:
Locum, 3-month contract
Shift Pattern:
Full time
Support with the review and management of the waiting list in the South Community Mental Health Team
Case review, telephone calls and face to face reviews with service users experiencing complex mental health challenges within a secondary mental health service
Completion of documentation, CPA/NCPA reviews, section 117 reviews, ward rounds, attendance at MDT meetings and ensuring safety planning, along with safe discharge from services
Requirements of the
Senior Community Mental Health Practitioner:
Registration as a qualified Nurse, Social Worker or Occupational Therapy
Sound risk assessment and clinical decision making skills
Assessment skills relating to complex mental health presentations
Experience of working within Adult Community Mental Health Teams is desirable
