We are seeking to appoint an enthusiastic, motivated, forward thinking Band 4 Assistant Practitioner/Nurse Associate to work within the District Nursing service working within a team, providing home-based care led by District Nurses.
The role is full time (37.5 hours) working across a 7 day week.
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams, with focus on client need.
This vacancy is based in the community division so you will need to be able to travel to Ashton, Wigan and Leigh.

Planning and Organisational Duties
- To undertake the delivery of nursing care as delegated by the DN team manager/Team Leader, for example, clinical skills such as catheterisation, phlebotomy, carried out in patient's homes.
- To work at all times in accordance with all Trust policies, procedures and protocols specific to the service.
- To maintain accurate patient care records in accordance with the Trust Guidelines for Records and Record Keeping.
- To be involved in aspects of progressive information technology, i.e. inputting data for system one and communication tools as appropriate practice, within the District Nursing Service.
- To contribute to the administration duties required by the service. For example, telephone enquiries, filing patient's notes.
- To ensure information is accurate and up to date for future reference and complies with Data Protection Act.
Compliance with the Health & Safety at Work Act the post holder is required to fulfil a proactive role towards the management of risk in all of their actions.
This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
