About The Roles:

Our Social Care and Support Service is committed to maximising strengths, to enable adults to achieve their desired outcomes, thereby meeting their needs and improving or maintaining their wellbeing.

We are looking for someone who shares our passion and is committed, enthusiastic, dedicated and passionate about improving outcomes for adults in Warwickshire supporting their discharge from hospital and onwards home through the Reablement service.


We are looking for someone with a passion to work in Intergrated Care be it in the acute hospitals, Discharge to Assess beds, Reablement Exits, Moving on Beds and Trusted Assessors services.


You will be able to demonstrate excellent organisational skills, a head for complexity, and a desire to improve the outcomes and wellbeing of those you work with upon discharge from hospital whilst supporting a team of Social Care practitioners, Social Workers and Trusted Assessors.

Hospital Team

The post will offer you the opportunity to:

  • Enhance your leadership skills
  • Develop and support a team of unqualified and qualified staff
  • Work with our health colleagues and ensure we maintain these excellent working relationships
  • Develop your own personal and professional skills across the whole of Intergrated Care
  • Be involved and lead on service developments whilst continuing to deliver a streamlined service
  • Continuously improve quality and compliance with current and new legislation.


As Team Leader you will be expected to deliver an effective day to day service to ensure timely and safe discharges of customers from hospital with the appropriate care and support at home, whilst working with colleagues in health/ residential home/ reablement and many more.
