    Job overview

    An exciting opportunity has arisen for a community rehabilitation band 6 physiotherapist to join a supportive and enthusiastic community ward therapy service as a Physiotherapist, working in in patient rehabilitation delivering rehabilitation and supporting discharge planning

    The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.

    We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions.

    You will be working with patients with a large variety of conditions and disabilities. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

    The service operates across 6 days Monday- saturday

    Main duties of the job

    The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. . Responsible for managing a caseload of patients with a variety of complex conditions including long term condition management and acute hsopital discharges. You will see patients in own homes , working with an MDT in other organisations. You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention .

    6 day working is in effect - Monday- Saturday and bank holidays on a rota basis

    • oPost graduate therapy experience to include community and rehab

    • oProfession specific Diploma/degree in Occupational Therapy or Physiotherapy oHealth Professions Council Registration oEvidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations. oMembership of the Chartered Society of Physiotherapy/ Royal College of Occupational Therapy oInvolvement in Special Clinical Interest Group
