    Job summary

    Were looking for motivated, enthusiastic and forward thinking registered staff nurses to work within our two In-patient wards that have 17 beds each for patients. Youll have experience of working in end-of-life care within an acute or palliative care setting. We will also consider newly qualified nurses who want to develop a career in palliative care. The successful candidates will be keen team players with good interpersonal and communication skills who enjoy working in a challenging environment whilst displaying a caring attitude at all times.

    You will need:

  • To be a Registered General Nurse and hold a Intra Venous administration Accreditation
  • The ability to plan, implement and evaluate care
  • Experience of working in end of life care within an acute or Palliative Care setting.
  • Continuation of NHS Pension Scheme is available.

    Main duties of the job

    Independentlyassess physical psychological, spiritual, cultural, religious, social, legaland ethical issues affecting the patient and carer.

    Plan anddeliver appropriate care in consultation with the multi professional team.

    Ensurepatients views are taken into account when planning care by establishing theircare needs and preferences both on admission and continually thereafter.

    Recogniseemotional distress in patients, relatives and carers and respond, using empathyand supportive communication skills and recognise when to refer on to otherspecialist practitioners within the care team.

    Perform careafter death according to St Josephs procedures and practice.

    2. Servicedevelopment

    Participate in collaborative decision making with allthose responsible for delivering care within the nursing team to ensure thatthe patients wishes and preferences are represented in every aspect of theircare.

    Keep accurate clinical records in accordance with StJosephs policy and procedures and Nursing and Midwifery Council guidelines.

    Safely administer prescribed medication and monitoreffectiveness in line with Nursing and Midwifery Council guidelines and StJosephs Hospice policy and procedures.

    Discuss with other members of the multi professionalteam individual patient needs; identifying appropriate strategies for symptommanagement and assessment in keeping withevidence based nursing care.

    Thispost holder is required to demonstrate competency in practice as detailed inthe St Josephs Hospice competency framework.

    Specialistpalliative care is delivered continuously over 24 hours and during thistimeframe, in the absence of the Ward Manager and Team Leader, the AssociateTeam Leader has the delegated responsibility for the overall management of theshift of the ward.

    Perform clinical procedures competently according toSt Josephs Hospice policy, procedures and practice.

    Plan admissions and discharges in liaison with colleagues discharge facilitator social workers, district nurses and community palliativecare nurses, to continue a high standard of palliative care, according to StJosephs Hospice policy, procedures and practice.

    In accordance with St Joseph's Hospice Health andSafety policies and processes be proficient in the use of manual handlingequipment, taking due care and paying attention when using all manual handlingequipment and resources.

    3. Linemanagement support

    Directlysupervise junior colleagues, student nurses and HCAs, performing care to ensuresafe practice in accordance with established evidence based nursing practiceand protocols to be aware of their specific training needs and objectives

    In the absenceof the Ward Manager or Charge Nurse be responsible for all aspects of clinicalcare. Ensure safe staffing levels and take responsibility for the health andsafety of all nursing, support staff and patients, taking advice from thehospice bleep holder on clinical or site related matters.

    4. Educationand Training

    Deliver teaching sessions in areas of expertise withinthe ward area, as agreed with the Ward Manager and Charge Nurse

    Act as a mentor to junior members of the nursing team,supervising their clinical practice including., health care assistants andnursing students

    Take responsibility for own professional developmentby keeping up to date and attendingrelevant professional/clinical courses both at local and nationallevels, in agreement with the Ward Manager.

    Develop an awareness of clinical supervision and theneed for self-reflection.

    Act as a positive role model to aid the development ofworkplace learning by ensuring open discussion and individual learning ispracticed by nursing students and unqualified staff in the clinical area.

    5. Quality

    Participate in quality improvement activities and makea contribution to the process of change

    Monitor own work to ensure to maintain quality byensuring that evidence based practice is adopted at all times.

    Participate in research, audit and quality initiativesas directed by the Ward Manger.

    6. Communication

    Negotiate and advocate on behalf of patients,relatives and staff, providing information that can be difficult for people tounderstand or can cause distress for example giving pre / post bereavementsupport, information, advice to patients, families, children, next of kin,friends and recognise the need to refer to other expert counsellors and otheragencies.

    Ensure effective channels of two-way communicationwith individuals, the multidisciplinary team, and other departments, outsideagencies, managers and staff. This cantake the form of receiving and acting on information, giving advice, andattending meetings.

    Adopt standards of behaviours in keeping with StJosephs Hospice mission and core value, the behavioural framework and NMCprofessional code of conduct.

    Communicate effectively face to face, by telephone, inwriting (including e-mail).

    Attend the multidisciplinary team meetings to providenursing opinion on patient and clinical issues ensuring that open and focuseddiscussion is practised.

    Meet with patients carers to address and advise onnursing care, clinical practice, and patient related inquiries.

    Attend regular meetings within the Hospice, forexample ward nursing staff/ multidisciplinary team handover, Health and Safetyand relevant meetings and briefings as directed by the Ward Manager.
