    Middlseborough Council are looking for a Specialist Nurse Prescriber to provide a range of clinical interventions within the substance misuse service, working in collaboration with the recovery caseworkers to delivery an effective, comprehensive package of care. The post-holder will be an integral member of a multi-disciplinary and multi-agency team delivering recovery orientated alcohol and drug intervention services.

    Purpose of the Post:

    Duties and Responsibilities:

    1. Diagnose and manage on an individual needs basis, integrating both drug and non-drug based treatment methods into a management plan, in conjunction with the caseworker.

    2. All relative prescribing practice including but not exclusively assessment, titration, review of therapeutic effectiveness, detoxification, relapse prevention.

    3. Prescribe and review medication for accordance with evidence based practice and national and local protocols, and within scope of practice.

    4. Identify and assess suitability of service users for in-patient alcohol and opioid detoxification as appropriate.

    5. Assess physical and mental health needs, facilitating access to appropriate interventions in collaboration with the caseworker.

    6. Proactively in the prevention, identification, and treatment of blood borne viruses.

    7. Support service users to adopt health promotion strategies that promote healthy lifestyles.

    8. To adhere to the medicines management guidance and local prescribing pathways on appropriate prescribing.

    9. Contribute to the effective and person centred care planning and risk management of individuals receiving treatment.

    10. Understand and apply legal framework/processes that support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable service users' health procedures and local guidance.

    11. Carry out additional duties as required by the Service commensurate with grade and experience.
